PERIODONTAL CARE
$635/yr Per Individual Enrollee
Services Include:
- 3 Periodontal Maintenance Cleanings/year (diagnosed periodontal management cleanings) with oral cancer screenings
- 2 Exams/year
- Up to 7 Bitewing radiographs
- 3 Periapical 1x/year
- Fluoride discounted to $35 (optional)Â
*Enjoy a 30% discount on all other in-office procedures/additional exams. If a 4th periodontal cleaning is needed in an enrollment period year, a 30% discount will be applied to that Cleaning.
Not ready to enroll? We offer 10% discounts to senior citizens (65+) and cash pay patients (excluding other promotions).
*30% discount excludes Invisalign treatment, Nightguards, Botox/Injectable treatments, or other promotions.
*No refunds for services not rendered within the calendar year.

Harbor Dental Discount Plans can not be combined with an insurance policy or other discount plans or promotions. These discounts are only available with the providers at Harbor Dental, 2726 Illinois Dr. #101, Fort Collins, CO 80525. Fees are non-refundable and can not be transferred to any other individual. All payments are due at the time of service. It can not be used at another dental office location. If dental treatment is required from a medical event, vehicle or other transportational accident, workman’s comp or disability claims, or any other situation where legal or another outside party is responsible for billing, the in-house discount plan can not be used.
All discount plan benefit periods are good for 12 consecutive months from the date of enrollment.
